Please bring a valid ID to your appointment. We need this to complete your registration.
The questionnaire is ready for you in the patient portal. You will receive an email invitation for this. The doctor will discuss this information with you during your first visit. If the questionnaire has not been completed in full, going through it takes extra time and a second appointment will be needed.
The questionnaire contains questions to give us a clear picture of your personal situation. This helps our specialists prepare properly for your appointment.
For your healthcare costs to be reimbursed, you need a valid referral letter² from your GP or specialist. Without this letter, you will be registered as a self-referred patient and your health insurer will not reimburse the costs. The referral letter, with name and signature, is valid for one year from the date of issue and must be dated on or before the date of your first appointment. In most cases, your GP will send the referral directly to us. You may want to check this with your GP to make sure everything has been arranged properly. So please arrange it in good time via Zorgdomein, or send it by email together with the completed forms. If we do not receive a referral letter, you will receive the invoice yourself³. After one year or after a pregnancy, you will need a new referral letter.
² In many cases, we need a referral letter before we can schedule an intake. This does not apply to treatments that are not carried out on medical grounds (such as social egg freezing).
³ If we receive the referral letter after we have already sent the invoice, additional administration costs will be charged.
Before your first visit, we also ask you to read, sign and return the document Consent to healthcare costs. You will receive this document after you register. Please make sure we have received the completed document back from you before your appointment. You can find our fees on this page.
If you have been treated elsewhere, we would like to receive your medical records from the healthcare provider concerned.
Request your medical records from the hospital/clinic where you were treated and check whether they will send the records directly to us. If you already have your records, you can email them to us yourself. See our contact page for more information about medical records from elsewhere.
We believe in a world in which everyone is free to start a family. We do this by supporting you and your partner (if you have one) in a personal, expert and professional way. Our approach has been carefully designed to provide the best medical care and support.
Your journey always starts with registration via our website. We then schedule a first consultation (intake consultation) to discuss and assess your personal situation. From there, we guide you step by step: from investigations to treatment and aftercare. We take the time to understand your situation and draw up a treatment plan that suits your situation.
